Living with or caring for someone with a chronic condition? Join us for the 2nd session of a 6-week Chronic Disease Self-Management Program developed by the Stanford University School of Medicine.

If you are an adult with a chronic condition, such as diabetes, arthritis, high blood pressure, heart disease, cancer, chronic pain or anxiety, or if you are a caregiver for someone suffering from chronic disease, this workshop can help you. Topics covered include: techniques to deal with frustration, fatigue, pain, and isolation; appropriate exercise to improve strength, flexibility, and endurance; decision-making tools; skills to communicate effectively with family, friends, and health professionals; nutrition information and action plans.

Workshops are free, but registration is required.
